If you wish for your prescription to be sent to your regular pharmacy please ensure to leave the name and address of the pharmacy on the form.

 

Please allow 72 hours for collection of requested prescriptions from your nominated pharmacy.

You can also collect a form from inside the entrance of the practice or download a copy for printing here . Completed forms can be returned by email, posted or dropped into the designated post box at the surgery.
