Sick Certificates

At our practice, we issue all medical certificates in accordance with our independent clinical judgment and our professional obligations as registered medical practitioners. If you need time away from work due to illness, we will certify the period that is medically appropriate based on the nature of your condition, your expected recovery, and our clinical assessment.

You may be entitled to Illness Benefit from the Department of Social Protection if you cannot work because you are sick or ill. To claim Illness Benefit, you must obtain a medical certificate called a Certificate of Incapacity for Work from your GP. Your GP completes the certificate and can submit it electronically through a secure online system. In some cases, your GP can issue a single certificate to cover the expected duration of your illness. Otherwise, you may need to return for further review. You can apply online to open your claim HERE.

Your employer may also require you to provide a separate medical certificate ('sick note/work cert' from your GP) while you are on sick leave. For example, you may need to provide a medical certificate if you are absent from work for more than 2 or 3 consecutive days. Where your recovery period is predictable and an extended absence is clinically appropriate, we will issue a certificate covering the appropriate duration. We do not routinely provide weekly medical certificates or ask you to attend weekly appointments solely to renew a certificate when there has been no change in your clinical condition.

Please note that you must see a GP to receive an Illness Benefit certificate or a medical certificate. If you need to extend either certificate, you may submit the form below. We will contact you if you need to be reviewed by a GP.

Illness Benefit

If you are eligible, we will submit your social welfare certificate electronically to the Department of Social Protection.

You must also complete and submit your own Illness Benefit application through MyWelfare. Your claim cannot be processed until your part of the application has been submitted.

You can complete your application online HERE through MyWelfare using your MyGovID account. If you do not have a MyGovID account, you can collect an IB1 (Illness Benefit Application) form from the practice to complete and send to the DSP.
